The Yadkin County Community Paramedic (CP) Program is an innovative healthcare initiative designed to bridge gaps in care, promote wellness, and support our most vulnerable community members. Our specially trained paramedics provide personalized care in homes and community settings, focusing on prevention and connecting residents with the resources they need to thrive.
How We Help Our Community
Chronic Disease Support
Our community paramedics provide in-home support for conditions like:
- Diabetes management
- Heart failure (CHF) monitoring
- COPD and asthma support
- Hypertension monitoring
- Medication management assistance
We work alongside your doctor to help you better understand and manage your condition from the comfort of your home.
Hospital Discharge Follow-Up
Recently discharged from the hospital? Our community paramedics can:
- Visit you at home within 24-72 hours after discharge
- Review your discharge instructions with you
- Check your medications to prevent errors
- Assess how you're recovering
- Connect you with follow-up care
We help ensure a smooth transition home and reduce the chance of readmission.
Substance Use Recovery Support
Our dedicated opioid response community paramedic offers:
- Compassionate follow-up after overdose events
- Connection to treatment and recovery services
- Naloxone distribution and education
- Support for individuals and families affected by substance use
- Confidential, judgment-free assistance

Senior Support Services
We help our older residents age safely at home through:

This is a no cost program to those in Yadkin County.
How It Works
Our community paramedics are State Credentialed paramedics with additional specialized training in community health. They work Monday through Sunday from 7:00 AM to 7:00 PM, providing non-emergency services that complement, rather than replace, your regular healthcare provider.
Community paramedics may come to your home by:
- Referral from your doctor, hospital, or other healthcare provider
- Referral from EMS after a 911 call
- Referral from Yadkin County Human Services Agency
- Direct request from you or a family member
